Bipolar Disorder: Mood Log

worksheet

When a client notices the warning signs of a manic or depressive episode early, they can preemptively take steps to control it. Completing a daily mood log is an excellent tool for catching these signs as early as possible.

The Bipolar Disorder: Mood Log is a simple and effective template for tracking the warning signs of bipolar disorder. Mood, sleep, and common symptoms of mood episodes are recorded each day to create a running log of changes. Because it’s important to track symptoms daily, we designed this handout to be as brief as possible, while still being informative.

With enough practice, many clients will learn to automatically recognize the warning signs of mania and depression. However, it's still recommended that a mood log is completed as part of a daily routine.
